In need of a medical appointment?

You can submit same-day or routine requests.
For same-day matters, submit the form on Monday to Friday between 8am and 11am. For routine medical requests, prescriptions, or any administrative inquiries, submit the form on Monday to Friday, between 1:30pm and 4:30pm. All routine requests will be responded to within 2 working days.
