Making an Appointment
If you wish to make an appointment with a doctor, please complete an online medical enquiry form. After an online medical enquiry has been completed, a named GP will triage and we will offer a face-to-face appointment or a telephone consultation as clinically indicated.
If you have an urgent request requiring a same day appointment with a doctor then please fill the online form in between 8am and 11 am when it is available.
For all routine GP appointment requests please complete the form in from 1:30pm to 4:30pm
In the afternoon if you have an urgent request requiring doctor attention please phone the surgery after 1pm.
For all other appointment requests not involving a doctor eg need nurse to dress wound, call the surgery on 01223 361611 or 01223 845185
Sickness Certificates (Fit Notes)
You must give your employer a doctor's 'fit note' (sometimes called a 'sick note') if you've been ill for more than 7 days in a row and have taken sick leave. This includes non-working days, such as weekends and bank holidays.
Home Visits
Home visits are for those who are too frail or unwell to attend the surgery. Requests for a home visit will be triaged by the practice nurses or a doctor.
If medically necessary the doctor will visit at the end of their surgery or sooner if urgent.
Please try to telephone the surgery before 10.00am so that the doctor can plan his/her visit schedule.
Change or Cancel an Appointment
If you cannot attend an appointment for any reason please inform us as soon as possible in order to enable us to offer the slot to another patient. Appointments can also be cancelled by the NHS app, completing an admin enquiry form on our online consulting tool or by contacting us via our landline numbers 01223 361611 / 01223 845185
